Permission Slip for Cooking Class

Hi everyone!

We've got a program comming up in late February (during vacation week) that features no-cook snacks. We are requiring permission slips for the program since it will involve food and will will probably be using knives.

I was wondering if anyone has done this sort of program before, and if there was a permission slip I could take a look at. We’ve got a very basic one that I am modifying, but I would like to see what others have used if I can!

I don't know if you need any legal jargon in your permission slips, but I spent a few years working in a very large CHI library (300k items in the CHI collection alone) and got to work on lots of "craft" projects tied in with story time. I love the idea! The only thing I can think of is you might wan to avoid peanut butter in case anyone is allergic. Ha! emoticon Sorry I'm of no help with the wording on your slips though.
