The Patron Instruction survey consists of a maximum of 18 questions. Depending on the responses provided, a respondent may not see all 18 questions.

Survey Questions
- Library Name, City and State.
- Over the past 12 months, has your organization offered informal, unscheduled, one-on-one patron instruction (e.g. assistance with basic Internet searching questions)?
- Over the past 12 months, has your organization offered in-person group patron instruction (e.g. classroom instruction with trainer present)?
- Over the past 12 months, has your organization directed patrons to freely available online instruction resources (resources that are NOT paid for by either your organization or a state/regional library)?
- Over the past 12 months, has your organization offered remote or online patron instruction (e.g. webinars, distance learning, teleconferencing, or online tutorials)? We are specifically interested in instruction that is fundedby either your organization, by a state/regional library or made available through partnerships with other organizations.
- You indicated that your organization does not offer remote or online instruction (e.g. webinars, distance learning, teleconferencing or online tutorials) for patrons. Below is a list of possible reasons for NOT offering remote instruction. Please indicate how important each of the following factors is to your organization’s decision not to offer this service.
