Use this worksheet to help keep track of your Interlibrary Loan statistics for the year so that you will be prepared to input your statistics on your year-end ILL report.

Public libraries will be submitting their ILL statistics in the Annual Report and Survey in the fall.
Academic and Special libraries will be submitting their ILL statistics in the same Survey Monkey report form as before, but the report will be a year-end (not monthly) report submitted in July only.
